Child & Teen Checkups are checkups for children and teens, up to age 21, who are enrolled in any of the Minnesota Health Care programs (Medical Assistance or MinnesotaCare). They help to keep children healthy and treat problems early. For more information on how to enroll, contact Benton County Human Services at 320-968-5087 and ask for a financial intake worker.

Where do I get Child & Teen Checkups?

  • Your doctor or clinic. Call your own family doctor for an appointment. Be sure to ask specifically for a Child & Teen Check-up.

Did you know?

  • Some health plans have incentive programs (such as Visa gift cards) for taking your child to the doctor for check-ups and/or immunizations. We can provide more information and vouchers on request.

Child & Teen Checkups include...

  • Complete physical exam
  • Immunizations
  • Information about good physical health
  • Information about thoughts, feelings and relationships with others
  • Questions and answers about your child's health and development
  • Shots
  • Hearing check
  • Vision check
  • Lab tests
  • Checks on growth and development
  • Routine dental care, usually beginning with the first tooth, and no later than age 1

 

You may use Child & Teen Checkups for any required checkups or physicals for Head Start, WIC, school, daycare, sports, and camps. Be sure to bring any forms you need completed with you to your child's appointment.

When should my child get Child & Teen Checkups?

  • Between birth and 1 month
  • At 2, 4, 6, 9, 12, 15 and 18 months
  • At 2, 3, 4 and 5 years
  • Every year after age 5 until age 21
