COVID 19 Vaccine Clinic Information

Registration Information: Any and all future clinic dates will be posted here. Future clinics will be based on the need in our community. If we have any upcoming vaccine clinics, they will be posted below.
Insurance information: the only information required on the health insurance information section is the type of insurance you have (private, public, Medicare, etc.). Once you select the type of insurance, go to the bottom of this page and click ‘Save and Continue.’
Review your Details/Summary Page: You must scroll to the bottom of this page and click on ‘Save and Continue’ or your appointment will not be saved. A confirmation email will be sent to those who successfully register.
On the day of your appointment: please arrive on time and bring your vaccine card if you have one. Masks are required in our clinics and one will be provided to you upon entry. The entire appointment time should take around 20-25 minutes, which includes a wait time of 15 minutes after being vaccinated to monitor for any adverse reactions. It is also important to eat/drink before your appointment.

COVID-19 Vaccine Booster Doses:
