COVID 19 Vaccine Clinic Information

Who is eligible for a COVID-19 Vaccine Booster Dose? 

  • COVID-19 vaccine booster doses are available for Johnson & Johnson vaccine recipients who are 18 years and older who received their initial dose at least 2 months ago. You can receive either a Moderna or Pfizer booster vaccine.
  • COVID-19 vaccine booster shots are available for Pfizer and Moderna vaccine recipients who completed their 2 dose series at least 6 months ago and are: 
    • 65 years and older
    • Age 18+ who live in long-term care settings
    • Age 18+ who have underlying medical conditions
    • Age 18+ who work in high-risk settings
    • Age 18+ who live in high-risk settings

If you believe you qualify for a COVID-19 vaccine booster dose based on the information above, please read the information below in its entirety. We are also accepting new COVID-19 vaccine patients for Pfizer vaccine (12 years and older).

Appointments are required. Please only register for one clinic option. This helps us plan for the amount of vaccine and supplies needed at the clinic.

REGISTRATION INFORMATION:

Insurance information: the only information required on the health insurance information section is the type of insurance you have (private, public, Medicare, etc.).  Once you select the type of insurance, go to the bottom of this page and click ‘Save and Continue.’

Review your Details/Summary Page: You must scroll to the bottom of this page and click on ‘Save and Continue’ or your appointment will not be saved. A confirmation email will be sent to those who successfully register.

On the day of your appointment: please arrive on time and bring your COVID-19 vaccine card and your face covering. Masks are required in our clinics. The entire appointment time should take around 25 minutes, which includes a wait time of 15 minutes after being vaccinated to monitor for any adverse reactions. It is also important to eat/drink before your appointment.
