Join Public Health and CentraCare staff for a community conversation about health in Benton, Sherburne, and Stearns Counties.
Central MN CHIP is using the Mobilizing for Action through Planning and Partnership process (MAPP) as part of the required Local Public Health Assessment and Planning process. At this meeting attendees will participate in the Forces of Change Assessment. It will identify all the forces and associated opportunities or threats that can affect, either now or in the future, the community and local public health system.

- Strategies to Overcome Barriers to Healthy Living
